  • Title: The Impact of Social Determinants of Health on the Custodial Journey of a Diverse Minority Group Member and Support Services in Place to Address Inequities Introduction: The criminal justice system is responsible for the detention, prosecution, and punishment

Choose a diverse minority group member and discuss their potential contact with the Criminal Justice System as it relates to their diversity. Describe their (potential or actual) custodial journey and the services that are in place within your organisation which can support the individual and their families.
    Focus your discussion on:
The lens of the Social Determinates of Health
Current health and justice statistics specific to this minority (using current data and evidence).
Your organisation’s support services and how they support this cohort (please use a pseudonym to protect your organisation)
Services both within and outside your organisation which may be accessed to support this cohort, while in custody and upon release (This includes how you are able to facilitate access for the individual, considering equity and access of minority groups to established services)
    Note:
A pseudonym must be used for the organisation and the individual receiving care
If you are not currently working in a custodial environment you can use a relevant custodial healthcare model currently used in your state or country of residence (there are Models in Week 7 you can explore)
Your assessment report is to be presented in formal academic writing style

  • “Improving Patient Safety Outcomes: The Importance of Monitoring and Reporting Data in Healthcare Organizations” Improving Patient Safety Outcomes: The Importance of Monitoring and Reporting Data in Healthcare Organizations In the current healthcare landscape, there is a growing emphasis on quality

Leadership at My Excellence Center became concerned when reimbursements began to shrink with the advent of pay-for-performance for Medicare patients. As they began to evaluate their performance metrics, they noted that there were large variations in outcomes across their patient care units. One safety outcome in particular, patient falls, seemed to show the greatest variability from month to month and across patient care units. Leaders determined to set improvement of falls as a priority for the organization.
A transdisciplinary team was formed to review evidence and make recommendations for practice change. The team reviewed evidence for acute care fall reduction programs, developed a protocol, and gained stakeholder support for its implementation. The team met to determine what outcomes should be measured.
    a. Why is it important to monitor outcomes?
    b. What key criteria must be taken into account when deciding what outcomes you will measure?
    a. In this scenario, what outcomes might the team measure?
    b. Thinking about the sources of data within healthcare organizations, where might the team find data to address the outcomes identified?
Simultaneously, leadership put a dashboard up to track progress and make data readily available to both unit leadership and staff to increase awareness and allow them to monitor data around the unit’s patient fall rates. Additionally, the dashboard allowed nurses to monitor potential relationships among patient falls, changes in staffing, volatility of admissions, discharges, transfers, and case mix. Nurse managers knew the literature suggested that there is a relationship between falls and staffing levels but were unsure how staffing levels were affecting their own units. With the implementation of the new dashboards on their units, they were able to evaluate factors surrounding times of patient falls in order to look for patterns. They became excited about their ability to monitor progress based on their own data.
What are some different mechanisms to report data to stakeholders in a meaningful, understandable way?

  • Pharmacological Management of Gastrointestinal and Hepatobiliary Disorders: Case Studies and Treatment Plans “Managing Shortness of Breath in a Patient with Chronic Bronchitis: A Treatment Plan and Monitoring Strategy”

Welcome to week 4!  This week we will apply pharmacology concepts to the gastrointestinal and hepatobiliary systems.
Symptoms of various gastrointestinal (GI) and hepatobiliary disorders often overlap, making diagnosis and treatment challenging. For example, symptoms such as nausea, vomiting, constipation, and diarrhea are non-specific and could also be the result of underlying medical history or current prescription drug use. As an advanced practice nurse, you could be potentially responsible for providing care to a patient who may present with non-specific symptoms related to the gastrointestinal and hepatobiliary systems.
When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (ie diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.
    ASSIGNMENT
Submit your Assignment by Day 7 of Week 4 (Sunday night, 11:59 pm ET)
Case studies are a useful way for you to apply your knowledge of pharmacokinetics and pharmacodynamic aspects of pharmacology to specific patient cases and health histories.
For your week 4 assignment, evaluate drug treatment plans for patients with various disorders and justify drug therapy plans based on patient history and diagnosis.
    To Prepare:
Review the case studies (attachment) and answer ALL questions.
When recommending medications, write out a complete prescription for each medication. What order would you send to a pharmacy? Include drug, dose, route, frequency, special instructions, # dispensed (days supply), refills, etc. Also state if you would continue, discontinue or taper the patient’s current medications.
Use clinical practice guidelines in developing your answers. Please review all Required Learning Resources. Use the Medscape app or website and JNC 8 to complete assignment.
Include at least three references to support each scenario and cite them in APA format. Please include in-text citations. You do not need an introduction or conclusion paragraph.

    reference list. —————
Cultural Considerations related to identified problem: How does your identified problem in association with your identified
culture affect these areas when caring for the patient? How would you care for your patient(s) with this problem and of this
culture? Must be represented throughout the presentation.
    Antepartum implications
    Intrapartum implication
    Postpartum implications
    Newborn implications
    —————-
    The Nursing Process
Two nursing diagnoses associated with your culture and problem must be incorporated into your PPT. • 1 diagnosis for mother and 1 diagnosis for newborn (may be listed as a “risk for” diagnosis). • 1 expected outcome for each nursing diagnosis • 3 measurable interventions to achieve the expected outcome. (No evaluation will be required for this presentation). • “As evidenced” may be included in your nursing process if you wish to show as a group you understand what would be present on a patient with your diagnosis but it is not required since you will not have an actual patient and your diagnosis, as stated previously, will be listed as a “Risk for……..” diagnosis.
