In today’s health care environment, the focus on major purchases, as part of the capital budget, has emphasized the need for newer and better technology. Although the need to invest in capital equipment and technology has been ongoing, the nursing workforce must also have a safe and pleasant work environment.
This assessment provides an opportunity for you develop a capital budget for a major facility improvement that will benefit patients, nurses, and the organization.
Most nurse leaders work with operating budgets, but will also have input into capital budgets. A capital budget is typically developed separately from the operating budget and is usually developed to purchase items that are often referred to as big-ticket items that will enhance the capacity or services of an organization. For example, capital budgets may cover land purchase for the future site of a new outpatient surgical center, construction of a new building or build-out for renovation of an already existing building, or replacement of old equipment.
It is important to remember that health care is also a major business. While for health care providers, patient care and safety are priorities, health care institutions must operate from a business model in order to survive financially. As nurse leaders or executives, you often have a role to play in developing a capital budget to improve care at some level. In addition, a rapidly changing health care environment requires nurse leaders who have the financial management and budgeting skills to ensure that large capital expenditures further the mission and goals of the organization.

You are the unit manager of a 50-bed, step-down unit, and it is time for you to prepare your annual capital budget. You have seen a significant turnover in your nursing staff in the past three years; a review of exit interviews and patient surveys indicates low nurse satisfaction, poor morale, and complaints of an antiquated and depressing work environment. You have researched the impact of a positive working environment on staff productivity and morale and decided to request a renovation of the nurses’ lounge as the main purchase in your capital budget. Practice Problem: Obesity related comorbidities are prevalent in primary practice patient populations. Application: This model focuses on individual characteristics and experiences, behavior-specific cognitions, and behavioral outcomes. Nurse Practitioners can use this model to develop personalized interventions that motivate patients to engage in health-promoting behaviors, such as regular exercise, healthy eating, and smoking cessation.

Good morning! My name is Amy and I will be your child’s nurse today. With your child being newly diagnosed with diabetes mellitus, I would like to go over your child’s nutrition and possible complications that could occur.
Maintaining your child’s blood sugar within the desired range is crucial if they have diabetes. You should monitor what your child eats on a daily basis. Your child will still be able to eat some of their favorite foods and sweets, but they will need to be in moderation. You will need to spread out their carb intake throughout the day and offer different foods that they can try. Carbs elevate blood sugar levels, so you will need to be mindful of how many carbs your child is getting (Mayo Clinic, 2022). Your body converts carbohydrates into sugar (glucose) after you eat. The blood sugar level rises as the glucose enters the bloodstream. A hormone called insulin facilitates the uptake of glucose by cells that they use as fuel for your body (Mayo Clinic, 2022). Some carbs are better than others. You should try to replace white flour and refined grains with whole wheat pasta, brown rice and oats and whole grain breads, cereals and crackers. Fruits, vegetables and whole-grain foods are excellent options since they contain fiber, vitamins, and other nutrients. Fiber can assist in maintaining blood sugar levels within a healthy range by slowing down the digestion and absorption of sugar (Mayo Clinic, 2022). When possible, you should choose a meal that contains whole grains, fresh fruit, lean protein, vegetables and healthy fats. Types of lean protein include chicken, fish, eggs, Greek yogurt, beans and lentils and lean beef. Vegetables that would be good to incorporate into your child’s diet would be broccoli, leafy greens, cabbage, asparagus and carrots. Starchy vegetables such as potatoes and corn should be given in moderation (Eating Smart When Your Child Has Diabetes (for Parents) – Nemours KidsHealth, n.d.). Your child should avoid sugary drinks. Water is of course the best option, but you could try infusing their water with fresh fruits and herbs to help give it flavor. You can also add a sugar free crystal light or tea (Eating Smart When Your Child Has Diabetes (for Parents) – Nemours KidsHealth, n.d.). You will want to make sure that your child does not skip a meal as this could cause an alarming increase or decrease in your child’s blood sugar. You will also be taught how to carb count as this will help you know how much insulin your child will need at meals. Compared to carbs, protein and fat have less of an impact on the body’s blood sugar levels (Mayo Clinic, 2022). Since it’s crucial that your child consume the right amounts of both fat and protein, you might need to count the amount of each in their diet. Excessive fat might make it harder for your child to maintain a healthy weight and raise their risk of heart disease (Mayo Clinic, 2022). We will put in a consult for a nutritionist to come speak with you so they can discuss how much fats and protein your child will need.
Complications that could occur as a result of diabetes mellitus are nerve damage, kidney damage, heart and blood vessel disease, eye damage and osteoporosis (Mayo Clinic, 2022). If their blood sugar is not under control, nerve damage typically develops gradually over a long period of time. The walls of the tiny blood vessels that supply your child’s nerves may be damaged by too much sugar. This may result in discomfort, burning, tingling, or numbness (Mayo Clinic, 2022). The many microscopic blood artery clusters in the kidneys that remove waste from your child’s blood can be harmed by diabetes. Diabetes raises your child’s chance of later-life illnesses like high blood pressure, heart disease, stroke, and restricted blood vessels (Mayo Clinic, 2022). Diabetes may cause issues with eyesight by damaging the retina’s blood vessels. Diabetes can lower bone mineral density, which raises the possibility that your kid will develop osteoporosis as an adult (Mayo Clinic, 2022). I know this can be a lot of information, so I have some handouts here for you on what we went over. I will give you time to look over them and then can answer any questions you may have and we can discuss a meal plan for your child.

I explored the clinical practice guideline titled “Management of Chronic Pain in Adults” (Guideline No. NG193) from the Agency for Health Care Policy and Research website. This guideline addresses the challenge of providing effective pain management for individuals suffering from chronic pain, a widespread issue impacting millions globally. Chronic pain is defined as persistent pain lasting more than three months, significantly affecting patients’ quality of life and daily functioning.
The guideline emphasizes the adoption of a multimodal approach to chronic pain management. This approach integrates various strategies including pharmaceuticals, physiotherapy, psychological interventions, and lifestyle modifications (Agency for Health Care Policy and Research, 2019). The rationale behind this recommendation underscores the multifaceted nature of chronic pain, encompassing physical, psychological, and social dimensions.
The recommendation is supported by Level 1 evidence, indicating systematic reviews of trial data or trials with low risk of bias. This robust evidence highlights the efficacy of adopting a multidisciplinary model in managing chronic pain effectively in clinical settings.
While reviewing the guideline, it becomes evident that not all recommendations are supported by the highest level of evidence. However, a majority are backed by Level 1 evidence derived from systematic approaches like the multidisciplinary model. In contrast, treatments such as acupuncture and herbal supplements rely more on expert opinions and small-scale studies, indicating varying levels of evidence across different interventions.
The inclusion of recommendations with diverse levels of evidence aims to provide a comprehensive framework that can be tailored to individual patient needs. Chronic pain management is complex and varies among patients; hence, guidelines incorporate a range of evidence to support personalized care approaches (Smith et al., 2022). It is essential for clinicians to critically evaluate the evidence and apply the most validated recommendations to ensure optimal patient care outcomes.
In summary, the Agency for Health Care Policy and Research’s guideline on “Management of Chronic Pain in Adults” advocates for a multidisciplinary approach supported by high-level evidence. Despite variations in evidential support across recommendations, the guideline offers a holistic framework to address the heterogeneous nature of patient needs in chronic pain management.
