    The PICO Question 
    After studying Module 1: Lecture Materials & Resources, and completing 500 clinical hours, students undoubtedly witness problems or gaps in healthcare.  The PICO question Population/ Patient Problem: (P), Intervention: (I), Comparison: (C), Outcome: (O) is to ask a searchable and answerable question that provides evidence to improve practice.  In a well-written discussion post address, the following:
    Describe a real-world, current problem in US psychiatric care.  This could be a problem that affects clinical decisions and may include a broad range of biological, psychological, and sociological issues.
    Formulate a PICO question based on the question chosen in the announcement.  You will use this question in each discussion for this term.
    Summarize the reason this PICO question is so important to advanced practice nursing.
    Submission Instructions:
    Your initial post should be at least 500 words, formatted and cited in the current APA style.
    Provide support for your work from at least five academic sources less than 5 years old. Visit Purdue OwlLinks to an external site. for specific examples on how to create in-text citations and cite references using the current APA format.
    Students will use this PICO question when creating the abstract and academic poster.

    Step 1: To begin preparing for your case study analysis first watch this simulation. Within the simulation you’ll be able to identify multiple roles interacting with the patient, at times in conflict.

    As you watch the case study video consider this summary of the case, but also include your own findings: 
    Personnel Involved: Nurse, OT, AT, PT, Health Administrator 
    Materials: Professional Code of Ethics, Scope of Practice (State = California), moral model of ethical decision-making  
    Case Scenario: A newly licensed nurse is delivering massage to a 22-year-old athlete-patient who recently underwent a shoulder reconstruction due to a sport collision-injury. The patient has requested massage to relieve pain. During the delivery of this massage, the physical and occupational therapist arrive to determine course of treatment. Another member of the healthcare team, the athletic trainer, observed surgery and has orders from the surgeon to perform scar tissue massage therapy. All healthcare professionals feel this is in their scope of treatment, how do we proceed? Who is reimbursable? Who should be providing care in this setting? Who needs to get involved, if anyone else? 
    Additional Points to Consider: The nurse is providing an inappropriate massage that is contraindicated at this stage of the patient’s soft tissue healing. The patient’s insurance has limited post-operative rehabilitation coverage. The PT, OT, and AT feel that each of them should be the rehabilitation discipline managing this patient’s case. Integrate strategies for conflict management of this interprofessional patient scenario to bring this scenario to a patient-centered resolution. 
    Step 2: Part II–Analysis: Resolving the organizational conflict with TeamSTEPPS® best practices.  
    As you have completed your coursework you have completed lessons, simulations, and readings on the various techniques, tools, and methods of applying TeamSTEPPS® principles. 
    Each of you is enrolled in a graduate program at USAHS. As part of your role as a graduate student, you will gain many skills in the program that will lead you to a career path to becoming leaders in your field. In your role as a current or future leader, you will face many challenges in your leadership decision-making. Part of those challenges include ethical dilemmas, differing opinions, team personality conflicts, role differentiation, and leadership discrepancies. All these issues impact the strategic vision of the organization and the organization’s culture, and indirectly or directly the patient’s outcome.

  • Title: “Differential Diagnoses and Treatment Plan for a Patient with Major Depressive Disorder”

    Differentials
    Differentials: Three to five differential diagnoses including pertinent patient presentation/symptoms that should be considered and must be plausible to match the patient presentation. The student should write in detail WHY they thought the patient met this criterion using the previous information presented in part one and how the diagnosis was ruled out. The final diagnosis should NOT be included in the differential.
    DIAGNOSIS

    Measurement of patient progress and Assessment Tools: List 2-3 measures that could be utilized for this patient in the future to measure their progress. Include the name of the test, information about reliability & validity, score ranges, meaning of high vs low scores, how often the instrument should be administered, and patient score is assessed during evaluation. Mnemonic devices, such as “SIGECAPS” are not published/validated tests.
    Psychopharmacology
    Psychopharmacology: Medication with dose-instructions listed, pharmacodynamics, and pharmacokinetics. A detailed explanation is provided for the choice of this medication over other medications in the same category.
    Diagnostic tests
    Diagnostic tests that are specific to the case are described with the rationale whether or not they are done. Lab/Diagnostic template in Appendix A. List each test separately and provide the rationale (CBC is separated by each test – hemoglobin, hematocrit, etc and what abnormals could mean and the possible connection to a psychiatric diagnosis)
    Psychotherapy
    Psychotherapy: In one to two paragraphs, list 2 or more theoretical orientations which could be used in the treatment of this patient and give 3-4 sentences describing each theoretical orientation and why the treatment could be beneficial. Such options could include Interpersonal, psychodynamic, EMDR, Rogerian, CBT, Jungian, etc. Use references other than Sadler or Wheeler.
    Psychoeducation
    Psychoeducation: Pertinent information regarding aftercare, legal issues, health promotion such as diet, medical, psychological testing, medications, diagnosis, black box warnings, and educational websites
    Follow-up
    Follow-up: Instructions to the patient for follow-up visits, emergency access, or diagnostics. Include safety measures if indicated.

    Chief complaint: weakness, depression and fatigue
    History of present illness: Diabetes, hypothyroidism and depression.
    Ms. Abigail Harris is an 86-year-old African American woman who presented in the ED with weakness and fatigue.
    Reports weakness and fatigue for last month, increasing over last 7 days. Ms. Harris is occasionally unable to get out of bed in the morning. The timing of fatigue is every morning. Reports that her fatigue is somewhat alleviated by lying down, and that she has slightly more energy in the afternoon. No known triggers.
    Reports feeling guilty about appearance and lack of social interaction last two weeks. Reports not leaving her house much. She reports a depression diagnosis, and her current depression screening shows her positive for moderate depressive symptoms.
    Underactive thyroid. 
    Medication: Levothyroxine, Venlafaxine, insuline
    Insulin aspart: 16 units, SC, TID mealtimes
    Insulin glargine: 45 units, SC, daily
    Levothyroxine: 50 mcg, P.O., daily
    Venlafaxine extended-release (Effexor XR): 150 mg P.O. daily
    Zolpidem: 5 mg, P.O., PRN at bedtime
