    topic of interest was finding research validating the effectiveness of exercise to reduce symptoms of depression.  The topic was of interest to me for the following reasons:.  Family history of depression on the paternal male side with completed suicide of my brother, 1st cousin and grandfather. In addition, throughout my profession, I have observed side effects to various antidepressant medications, abuse of antidepressant medications by mixing with other substances, and non-adherence to medications as catalysts to worsen depressive symptoms.
    What outcomes would you like to see emerge from this?
    Some individuals do not enjoy exercising, furthermore, individual response to exercise can vary with different exercise modalities. The desired outcomes would be for further research to validate the effectiveness of RewP testing.  Further research on the effects of acute and chronic exercise on brain function by measuring event related potential (ERP)  would also be helpful in validating positive effects of exercise. ERP’s are small electrical potentials from the cortex of the brain, they can provide information about brain activity such as higher level processing (thinking) and sensory perception processes such as learning, memory, emotions and expectations.(Gusatovic et al., 2022). Further evidence supporting the validity of the RewP as a predictive measurement tool supporting the positive effects of exercise, may lead to more providers utilizing the equipment, thus hoping the cost of testing would decrease. If more providers were able to use the RewP as a measurement to predict positive response, they would be able to customize the right modality of exercise to obtain maximum results.
    How would you measure these outcomes?
    I would utilize tools like those used by (Brush et al., 2020).  EEG-flankers and ERP measurements for RewP measurements.  Reward positivity, (RewP) is a measure thought to be correlated with the positive reward processing center of the brain. The results of the study suggested a greater (RewP) predicts a response to exercise for adults with major depression (Brush et al., 2020).   In addition, I would use self-assessment tools and questionnaires, such as the Mini International Neuropsychiatric Interview, to obtain subjective data pre and post exercise.
    Include in your discussion an analysis of how Aiken measured her selected outcomes.
    Patient mortality, patient surveys, care quality, patient safety, and nurse reported outcomes such as burnout and job satisfaction were the outcomes measured. Representative samples were gathered from hospitals over 6 countries. Patient mortality data was pulled from 188 hospitals, nurse reported data was pulled from 243 hospitals and patient ratings were pulled from 182 hospitals (Aiken et al.,2016).  An analysis was performed on the data pertaining to outcomes such as patient discharge, adverse events, hospital characteristics, as well as feedback from patients and nurses (Aiken et al.,2016).

    My topic for my peer reviewed article is titled Six-Month Psychopathological Symptom Trajectories following the Covid-19 Outbreak: Contrasting Mental Health Outcomes Between Nurses and the General Population. My goal is to become a mental health practitioner and assist those with anxiety, depression, and post-traumatic stress disorder, focusing on healthcare workers and the general population. For those of us who are working in the healthcare field and experienced covid, have most likely come across new mental health issues from the pandemic.  The peer-reviewed article was intriguing to me because it provides a contrast between healthcare workers and the general population. “The main findings showed nurses reported higher levels of depression, anxiety, and trauma than the general population” (Vitorino et al., 2024).  Being able to help create resilience-promoting interventions, but tailoring them to unique needs in each group. Organizational leaders have a responsibility to advocate for the mental and physical well-being of health care staff, from safe and resilient working environments and promoting social connections among colleagues. 
    To find my topic I accessed Walden University Library database. Cumulative Index to Nursing and Allied Health Literature (CINAHL) and National Libary of Medicine (NLM) are the two online resources I have found to be useful in my academic studies. CINAHL is a private corporation and provides the criteria for indexing journals”high impact factor; usage in reputable subject indexes; peer reviewed journals covered by other databases; top ranked journals by industry studies; and article quality” (Oermann et al., 2020).  This can give one the ease of knowing when researching using these databases that they are getting quality research. A difficulty I found was trying to narrow down my research to my liking. I did find too many words would result in no findings, using fewer words and different wording would narrow down the results. Another advantage was being able to click a box for peer-reviewed articles only, but then would find the issue of proper wording to narrow down the research. Overall, CINAHL and NIL are elite databases to use if you want high quality research-based articles.

    Search Databases
    This week’s discussion broadened my knowledge of the importance of searching for evidence-based articles in writing for my career. According to Al-Jundi (2017), using evidence-based articles helps integrate the article’s information into clinical experience and patient values (Al-Jundi,2017). The Al-Jundi (2017) article further states that “the simple process for assessing the credibility of articles and their value to your clinical practice” (p.2).
    I used CINAHL, Medline, and Comprehensive Psychiatry in the Walden Library to search for this article (Walden University Library, n.d.).  One of the challenges during my search for peer-reviewed articles was figuring out what keywords to use to narrow the search. At first, once I clicked the search button after putting in the topic, the page took me to the video on how to use the advanced search. I watched that video several times while trying different keywords that would yield results. For this week’s discussion, the topic of interest is the treatment of major depression using vagus nerve stimulation therapy versus other antidepressant treatments. The article of Vlaicu and Bustuchina Vlaicu (2020) discussed the efficacy and safety of using nerve stimulation therapy. The results from this article show that treatment-resistant depression who used vagus nerve stimulation therapy show both acute and long-term benefits such as safe response and excellent compliance (Vlaicu & Bustuchina Vlaicu, 2020).
    However, I chose this topic because of the increase in depression in the patient population I have encountered in my career. The rate of depression in patients who have other chronic illnesses is alarming.  I assessed the database A-Z: Nursing; I got to the Walden library page, checked the CINAHL with full text, and put the treatment of depression in the first search box, vagus nerve therapy in the second search box, and finally, antidepressant treatment. I also limited my search to peer-reviewed articles from 2017 to 2024. I found relatively great articles on my topic of interest. Then, I searched to compare journal articles comparing CINAHL and Medline; however, CINAHL produced twenty-five relevant articles, while Medline produced twenty-one relevant references. However, most of the articles helped determine the topic of interest.
    Surprisingly, I recommend both search databases, as one always must determine which of the searches will give one the best references for a topic of interest. One can use different parameters to drive a better reference based on specific interests, such as the source date, age, keywords, and more.
    I also searched using comprehensive psychiatry to find the topic of choice, where I found relevant articles that also discussed the methods used to implant the device used to stimulate the nerves and how vital the use of electronic devices is for the treatment of depression, especially in cases that are treatment resistant. (Bottomley et al., 2020).

  • “Addressing Performance Concerns and Implementing Improvement Strategies for Nurses: A Mindful and Collaborative Approach”

    In the case in which an experienced nurse is 60 days post hire and requires an upcoming performance review in which a performance concern must be addressed, I would first bundle the conversation mindfully. I would initiate the conversation with discussion on a positive contribution the employee has made, and secondly, address the performance concern. I would be careful to approach the finalization of the discussion with another positive quality or contribution the employee has displayed, and encourage the employee to continue to improve and pursue excellence. If the performance concern was related to a medication error, for instance, I would reeducate/remind the employee on the proper 6 rights of medication administration. I would speak with the nurse about how significant it is to be mindful of these rights to induce a proper outcome of medication administration. Without following this nursing standard, patient safety, quality, and person-centered care could be impacted. 
    As stated, a plan for an improvement in this scenario would be to have the nurse teach back the six medication rights and display performance of them while a supervisor checks off on the nurse. Behavioral expectations, in regards to demonstrating proper safety checks prior medication administration would suffice to offer resolution of the concern. Leadership competencies including communication, collaboration, interpersonal skill, and emotional intelligence would all be at play in the above scenario in order to effectively carry out the corrective and desired change of behavior in the employee. Cultural humility would also be required from the nurse leader to aid in effective change. It is a challenge to keep self care as a priority as a nurse, however, one must lead change from within first, and this takes great self reflection and great self care to be a leader. 
    Information technology could support performance management surrounding this issue by ensuring a scanning in process of each right of medication confirmation prior granting the medicine. The administration of the medication would be filtered through a precise standard of checks and balances in the scenario above with the 6 rights implemented in the scanning system prior moving forward with medication administration. A programmed pyxis would also support a safer process to support against error. Programming the pyxis with reminders to perform the 6 rights would be beneficial, as well as team emails sent out reminding the nurses to be careful with the process of medication administration.
