Correlation is the process of establishing a relationship between two or more factors. Situation:
Mr. Ramirez, Room 208. A 65-year-old man with left-sided cardiac failure.
Background
With increasing shortness of breath and weariness, Mr. Ramirez was admitted two days ago. His past includes hypertension and coronary artery disease. On room air, blood pressure is 142/90 mmHg, heart rate 112 bpm, respiration rate 24/min, and oxygen saturation 92%. BNP levels have risen in recent lab tests.
Assessment
Mr. Montero’s symptoms and increased BNP suggest left-sided heart failure is worsening. Exertion causes dyspnea and auscultation shows lung crackles. He has orthopnea and tiredness.
Recommendation
I recommend giving Mr. Ramirez intravenous diuretics like furosemide to minimize fluid overload and symptoms. Please also consider an echocardiography to better examine his left ventricular function. To manage heart failure, we may need to alter his ACE inhibitors and beta-blockers. I’ll keep an eye on his vitals and supportive care. Please come place orders.
