Imagine you are part of a global health organization tasked with improving health in a low-income country. What health determinant would you focus on first, and what strategies would you employ to address it? How would you measure the success of your intervention?

Andy is assigned to care for a 14-year-old girl, Amanda, admitted with a large tumor located in the left groin area.  During an assessment, Amanda shares her personal feelings with Andy.  She tells him that she “feels different” from her friends.  She is ashamed of her physical development because all of her girlfriends have “breasts” and boyfriends.  She is very flat-chested and embarrassed.  Andy listens attentively to Amanda and helps her focus on some of her positive attributes and talents.
A computerized tomography (CT) scan is ordered and reveals that the tumor extends to what appears to be the ovary.  A gynecological surgeon is called in to evaluate the situation.  An ultrasonic-guided biopsy is performed.  It is discovered that the tumor is actually an enlarged lymph node, and the “ovary” is actually a testis.  Amanda has both male and female gonads.
When the information is given to Amanda’s parents, they do not want her to know.  They feel that she was raised as “their daughter”.  They ask the surgeon to remove the male gonads and leave only the female gonads.  That way, “Amanda will never need to know.”  The surgeon refuses to do this.  Andy believes the parents should discuss the situation with Amanda as they are denying her choices.  The parents are adamant about Amanda not knowing anything.  Andy returns to Amanda’s room and Amanda begins asking all types of questions regarding the tests and the treatments.  Andy hesitates before answering, and Amanda picks up on this, demanding he tell her the truth.

Select one of the three incidents from the Vila Health: Patient Safety simulation. These are common incidents you are likely to encounter in the health care field. These included a patient identification error, a medication error, and a HIPAA/privacy violation. You may select one of the incidents you worked with in the previous assignments or select another one. Choose one that holds the most interest for you.
Consider the following analysis questions once you have selected the incident on which you will focus:

Write an analysis and leadership action plan for the issue you selected that will enable you to address the issue on an organization-wide basis. Please make sure to include all of the following headings and answer all of the questions underneath each heading.
Introduction: Issue Summary
How would you summarize the key elements of the incident that occurred?
What is your goal in addressing the issue?
Which 2–3 key items will be your focus? For example, you may elect to focus on nursing staffing levels if being short-staffed in nursing is contributing to compromises to patient safety.
Culture: Explain what culture is and why it is a critical priority for safety and quality, providing at least two evidence-based strategies for cultivating a culture of safety.

What is your rationale for selecting these departments? For example, you may want to involve nursing because many of errors involve nurses and obtaining their buy-in is critical to achieving the organizational priority.
Which specific senior leader, front-line staff member, and clinical expert will you include in your action plan and hold accountable for implementation?
What are the implications of not engaging with all departments toward making safety and quality top of mind?
How might you involve other departments in addressing the specific issue and the cultural issue?
Which specific leaders within the organization could assist you in addressing this issue and in making patient safety and quality top of mind throughout the organization? In addition, your assignment needs to conform to current APA style and format guidelines. Do ensure that it is clear, persuasive, concise, organized, and without errors in grammar, punctuation, and spelling. Do provide citations and title and reference pages in current APA format. Other leaders in your organization are going to want to know what sources you relied on to prepare your analysis and action plan.
Note: Remember that health care is an evidence-based field. You will need to cite a minimum of two credible references to support your analysis and action planning process.

If managers could have just one wish, many would ask for a crystal ball. With this tool, there would never be any worry about risk. The manager could look into the crystal ball and know exactly what will happen with each decision. Unfortunately, we do not have this luxury and must use other tools and techniques to determine the risks we face for the decisions we make. Understanding the financial risks will be the focus of this week’s discussion question.

In this module’s readings and on the Baldrige website, it is suggested that a commitment to quality begins and ends with leadership. Discuss the steps that a leader would need to take to implement a change in overall quality. As a suggestion, some of the steps covered in your paper could include initial identification of poor quality, who should be involved in the improvement process (management, customers, consultants, etc.), how to get employee buy-in, level of oversight required and by whom, implementation, etc.
