  • “Assessment, Diagnosis, and Treatment of Major Depression: A Clinical Case Study”

    CLINICAL CASE STUDY
    As per your Syllabus:
    This
    is a new feature for this class. Towards the end of the semester, I
    would give you a clinical scenario and I will like for you to write a
    2-3 page summary of your assessment, diagnosis and treatment
    recommendation. This needs to include the following:
    The
    methods and strategies you would use in order to perform the initial
    assessment. In other words, I want to know how you arrived to the
    diagnosis and what processes you used.
    Which
    diagnoses would you consider? You should have a primary diagnosis, but
    perhaps there may be other possible diagnoses you may want to rule/out
    or consider.
    What
    is your case formulation? That is more comprehensive than just the
    diagnosis. For example let’s say you are considering ‘Major Depression”
    as a Diagnosis. Your case formulation may be something like this: “this
    patient has suffered significant recent loses in his life, and in the
    context of possible biological vulnerabilities (ie; history of maternal
    depression) and limited psychological resources he has developed a
    depressive condition”.
    What
    is your treatment plan? Nothing extensive here but it has to make
    sense. Don’t just put things in there to make sure you cover all bases.
    Not all patients need MRIs!
    What
    else would you have liked to know about this patient, which was not
    given to you in the case scenario, and you think it may have been very
    useful in order to reach a diagnosis and develop a treatment plan? For
    example, the patient with depression has complained primarily of
    fatigue, mild dizziness and difficulties concentrating. Perhaps you may
    want to rule out a medical condition (anemia) and you may want to have
    this patient be medically evaluated.
    This is not a difficult task, but requires a little thinking from your part. As
    long as you are “in the ball park” for the diagnosis, you will be fine.
    The important point is that I need you to show me you know how to do
    the assessment, followed by a diagnosis, good case formulation and a
    reasonable treatment plan. Don’t write more than 3 pages (about
    1000-1200 words). You will have this task towards the end of the
    semester (see schedule below) and it will be worth a total of 10 points.
    As with any other assignments, this is your own work, not a team
    effort. Sharing or copying another student’s work will result in a
    failing grade for the class.

    a two-step process, students will evaluate and synthesize the external
    evidence results from the strongest external study retrieved during the
    search of the literature that supports the intervention of the PICOT
    question. – This
    is the study that answer the chosen PICOT question and directly relate
    to the chosen intervention. The study must be within the discipline of
    nursing and not medicine.
    – Assignment Criteria:
    Develop a scholarly paper that addresses the following criteria:
    1.Complete the Evaluation Table Template with the results from the strongest external evidence study. 2. Determine the strength of each of the study using the rating system in Melnyk and Fineout-Overholt (4th edition) page 18 and add the Level for each study to the evaluation table.
    a. Information on the evaluation table should be in bulleted format, not sentences
    3. Include a legend on the evaluation table.
    4. Place the completed Evaluation Table as an appendix.
    5. Write a one-page narrative that analyzes how the evidence from the two studies supports the intervention of the PICOT question. If the evidence does not support the intervention, describe what would be the next step in the evidence-based practice process.
    6. Include a brief introduction, purpose statement and conclusion. 7. Level 1 headings required.
    8. Include a title page, reference page and appendix.
    9. Write the paper in third person, not first person (meaning do not use ‘we’ or ‘I’) and in a scholarly manner. To clarify: I, we, you, me, our may not be used. In addition, describing yourself as the researcher or the author should not be used. 10. APA 7th edition format is required.

    Case StudySarah Green has been an accountant for 12 years. She worked in a small medical practice for two years after earning her BS in Accounting and then chose to work freelance for five years. For the past five years she worked at a large firm, Stokely and Associates, where she brought in more new clients than 92% of her colleagues and received a higher satisfaction score from clients than 90% of her coworkers.
    Sarah had to move to a new city to care for her sick mother. This meant finding a new accounting position. She was excited to be hired at the large accounting firm, Parker and Mendez Accounting, LLC, right away. Unfortunately, she learned after 4 months that she was being paid less than her coworkers who had equal or less experience than she did. When she asked the hiring manager about the discrepancy, she was told that the organization’s policy is to pay in a range based on previous pay. The hiring manager did admit to being surprised that her previous salary was at the low end of the expected range given her successes and performance reviews, but the policy is there for a reason and making an exception defeats the purpose of the policy.
    After researching her industry and old employer, Sarah learned that her previous employer regularly paid women less than men even when the women had more experience or brought in more clients. Her current pay was impacted by this discriminatory practice.
    The report to the VP of HR should include two parts:
    Part OneA summary of Sarah’s case.
    Address which of the EEOC acts the previous organization violated.
    An explanation of the ethical and legal obligations of the current organization.
    A proposed solution for Sarah.
    A proposed change in policy for determining starting pay at the current organization.
    Part TwoAn outline of a training session for all hiring managers and benefits and payroll specialists. Each of these sections should be included and explained briefly but with enough detail that the decision-makers reading the report are fully informed and can approve the training.
    Introduction: Provide an overview of the training. Example: Excellence in Interviewing is a two-week online training that introduces the best practices in interviewing.
    Scope: State how broad or narrow the focus of the training will be.
    Objectives: List what participants will be able to do using “To + verb” statements. Example: To roleplay an effective phone interview. To write three behavioral interview questions that assess given criteria.
    Training Method: Explain the format of the training. Online, face-to-face, blended, asynchronous or in real time or both.
    Evaluation using Kirkpatrick’s Levels: Address each level of Kirkpatrick’s in detail. Share specific strategy to evaluating the proposed training.
