“Preliminary Care Coordination Plan for a Selected Health Care Problem: Addressing Physical, Psychosocial, and Cultural Considerations and Identifying Available Community Resources” Title: Care Coordination Plan for Patient with Chronic Illness: Utilizing Community Resources for Improved Health Outcomes
Develop a 3-4 page preliminary care coordination plan for a selected health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.
The first step in any effective project is planning. This assessment provides an opportunity for you to strengthen your understanding of how to plan and negotiate the coordination of care for a particular health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.
As you begin to prepare this assessment, you are encouraged to complete the Care Coordination Planning activity. Completion of this will provide useful practice, particularly for those of you who do not have care coordination experience in community settings. The information gained from completing this activity will help you succeed with the assessment. Completing formatives is also a way to demonstrate engagement.
Scenario
Imagine that you are a staff nurse in a community care center. Your facility has always had a dedicated case management staff that coordinated the patient plan of care, but recently, there were budget cuts and the case management staff has been relocated to the inpatient setting. Care coordination is essential to the success of effectively managing patients in the community setting, so you have been asked by your nurse manager to take on the role of care coordination. You are a bit unsure of the process, but you know you will do a good job because, as a nurse, you are familiar with difficult tasks. As you take on this expanded role, you will need to plan effectively in addressing the specific health concerns of community residents.
To prepare for this assessment, you may wish to:
Review the assessment instructions and scoring guide to ensure that you understand the work you will be asked to complete.
Allow plenty of time to plan your chosen health care concern.
Complete the following:
Select one of the health concerns in the Assessment 01 Supplement: Preliminary Care Coordination Plan [PDF] Download Assessment 01 Supplement: Preliminary Care Coordination Plan [PDF]resource as the focus of your care coordination plan. In your plan, please include physical, psychosocial, and cultural needs.
Identify available community resources for a safe and effective continuum of care.
Document Format and Length
Your preliminary plan should be an APA scholarly paper, 3–4 pages in length.
Remember to use active voice, this means being direct and writing concisely; as opposed to passive voice, which means writing with a tendency to wordiness.
In your paper include possible community resources that can be used.
Be sure to review the scoring guide to make sure all criteria are addressed in your paper.
Study the subtle differences between basic, proficient, and distinguished.
Supporting Evidence
Cite at least two credible sources from peer-reviewed journals or professional industry publications that support your preliminary plan.
Grading Requirements
The requirements, outlined below, correspond to the grading criteria in the Preliminary Care Coordination Plan Scoring Guide, so be sure to address each point. Read the performance-level descriptions for each criterion to see how your work will be assessed.
Analyze your selected health concern and the associated best practices for health improvement.
Cite supporting evidence for best practices.
Consider underlying assumptions and points of uncertainty in your analysis.
Describe specific goals that should be established to address the health care problem.
Identify available community resources for a safe and effective continuum of care.
Organize content so ideas flow logically with smooth transitions; contains few errors in grammar/punctuation, word choice, and spelling.
Apply APA formatting to in-text citations and references, exhibiting nearly flawless adherence to APA format.
Write with a specific purpose with your patient in mind.
Adhere to scholarly and disciplinary writing standards and current APA formatting requirements.
Before submitting your assessment, proofread your preliminary care coordination plan and community resources list to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your plan. Be sure to submit both documents. -

“Maximizing ROI on Sponsored Search Ads: A Case Analysis and Recommendations”
Case Questions for Measuring ROI on Sponsored Search Ads
Instructions:
This exercise is to be completed individually. You may discuss the assignment in a group, but
each person should complete the exercise separately.
Please provide a clear, concise, and well organized essay that addresses at least the following
questions. You are free to address other issues in the case as well. The intent of the assignment
is to have you think critically about the marketing problems faced in the case. It is important
that you consider both sides of the argument when you answer these questions. Analyze the
quantitative material, if any, in the case to support your answers. Spend most of your time in
defining and defending your recommendation for what should be done.
Good answers may require assumptions of facts that may not be presented in the case. You
are welcome to make these assumptions, but please state these assumptions and briefly justify
why that are reasonable. Also, you may use whatever resources you can locate to provide
further information about this industry or the web in general. Please reference your sources.
Your response must be typed, double spaced, with one-inch margins, and a 10 to 12 point
font size. This writeup must not exceed 3 pages in length. You may attach exhibits, tables,
and/or graphs to support your arguments. These supporting materials must be referenced in
the text and do not count toward the 3 page limit.
Required:
e-Marketing focuses on making marketing more “accountable”. In other words, we want to use
quantitative analysis to direct our marketing expenditures and to get the best possible return from
these investments. A common metric for doing this is Return on Investment (ROI). ROI is the ratio
of return (=Return – Investment) to Investment. In our problem the investment is the cost of the
advertising, and the return is the profits earned from the advertisement. Please read the case
“Measuring ROI on Sponsored Search Ads” and answer the following questions.
1. What would happen if sponsored ads are removed from the website? How many would
click through to the website anyways?
2. Repeat Bob’s ROI calculation of 320% for sponsored search advertising for branded
keywords on Google. What assumptions is he making? Are these assumptions possibly
incorrect? Please explain your reasons.
3. What analysis can you do from the data to get a better ROI calculation for sponsored search
advertising for branded keywords on Google? Please explain any assumptions you make
based on the discussion in the case, conduct the proposed analysis and report the ROI you
have calculated. (Hint: Consider looking for a natural experiment that would tell you what
would happen if you did not invest in the advertisements.) -
Title: Developing a Thesis and Paper Outline for a Research Paper
It is time to start developing your ideas for your Research Paper. We will begin this process by creating a draft thesis and paper outline. The first step is your thesis statement.
STEP ONE: Developing the Thesis Statement
Your thesis should add a problem, argument, and/or opinion to your topic of interest. Include: a) your specific subject b) an opinion c) a blueprint of reasoning.
Avoid arguments like:
“Summer is hot and people should drink water” (too obvious), “We should add a houseplant to main entrance” (not interesting/relevant. What’s the purpose? Is there a need/urgency?) “Health care is too expensive” (too broad. focus on a specific program, location, population)
“Feather-blowing is a unique, but effective treatment program for veterans with asthma who grew up on farms” (Very obscure, are you going to find enough scholarly sources to effectively research this topic?)
“100% of hospitals should have free MRI scans to anyone who wants them” (nice idea, but is this realistic? Work with reasonable resources).
Thesis example: “Engaging people with Alzheimer’s through reminiscence-based movement programming is a low-cost tool that is beneficial for our participants, our communities, and our bottom line.”
This thesis includes a specific topic which includes a specific program and population. It is interesting, but not obvious or trite. Right away, the audience knows the opinion and purpose of the author. This can easily tie back in to an organization’s mission statement (serving the community, etc). It is realistic (low-cost). There is a blueprint which will serve as organization for the final proposal – (benefits for 1) participants, 2) community, 3) bottom line).
For more information on writing a thesis statement, please see OwlPurdue: https://owl.purdue.edu/owl/general_writing/the_writing_process/thesis_statement_tips.html
STEP TWO: Drafting a Paper Outline
Why create an outline?
Aids in the process of writing
Helps you organize your ideas
Presents your material in a logical form
Shows the relationships among ideas in your writing
Constructs an ordered overview of your writing
Defines boundaries and groups
How do I create an outline?
Determine the purpose of your paper.
Determine the audience you are writing for.
Develop the thesis of your paper.
Drafting the outline:
Brainstorm: List all the ideas that you want to include in your paper.
Organize: Group related ideas together.
Order: Arrange material in subsections from general to specific.
Label: Create main and sub headings.
Basically, your outline will constitute three main parts namely the Introduction, the Body and the Conclusion! Instructions:
Create an alpha-numeric full-sentence outline to your research paper. Include your: 1) thesis, topic, 2) keywords, 3) introduction, 4) 3-5 supporting ideas in the body of your discussion, and 5) point for your conclusion.
Use this structure of the Research Paper Outline document as a guide (included as an attachment) . You can change this format as long as you include the sections discussed above. Create an original document for submission. Paper Outline Template- Fall 2019.docx -
“Preserving and Revitalizing Native Cultures of Alaska: A Research Proposal and Analysis”
***NOTE***** I have a complete research proposal with outlined focuses of the paper and works cited, research already done with properly cited sources for your reference. I will include that as a file on the order. Final paper can be based on any aspect of Native Cultures of Alaska. Approaches can include a topic area that we have discussed over the semester, another topic that you are interested in learning more about, or something you and your family have had personal experience with. You must cite six sources, up to two of these can be personal communications you have with family or community members who have life experiences which relate to your chosen topic.
Length: Is your paper a full seven pages, up to 8 pages?
Format: Does your paper follow the guidelines for font, font size, margin size, and text spacing?
In-text Citation: Did you use an in-text citation from your textbook and articles from blackboard/other academic sources? Is your in-text citation formatted according to Chicago style guide? Did you appropriately use in-text citations for all of the sources listed on your reference cited page? Did you use an in-text citation every time you use outside information in your paper?
References Cited: Do you have a properly formatted (Chicago style guide) references cited page? Are your sources appropriate peer reviewed text (Note that any of the articles from Canvas would work as peer reviewed articles)?
Content: Is it obvious to the reader that you read the articles? Did you use specific examples from the articles book to support your arguments or discussion? Did you critically think about the articles? Did you apply concepts addressed in lectures, discussion board, films and articles? Do you personally reflect on the readings in your review? Make sure you are writing more than just a summary of the articles and are actually applying anthropological concepts in your paper.
Clarity: Did you use proper spelling and grammar throughout your assignment? Does your assignment follow a logical format (i.e. introduction, body, conclusion)? Is your entire assignment tied together by a cohesive idea or theme? Is your paper easy to read and understand? Are your points and arguments clear, how well does your entire paper flow together?
Citations – In order to cite properly you must cite in text as well as at the end of the document in a “References Cited” Section. You will likely learn many ways to cite sources while in university. For the purposes of our class please use the following format:
Example for in-text citation of article in journal
Any time you use an idea or information from an author you need to have an in-text citation at the end of the sentence or within the sentence. This should also be noted in the references cited section.
For example, using my article on Tohono O’odham traditional foods:
(Author’s Last Name date and page) =
There are many two organizations working to increase consumption and production of traditional foods on the Tohono O’odham Nation (Fazzino 2008, 40). OR
Fazzino (2008, 40) notes that there are many two organizations working to increase consumption and production of traditional foods on the Tohono O’odham Nation References Cited
Fazzino, David. 2008. “Continuity and Change in Tohono O’odham Food Systems: Implications for Dietary Interventions.” Culture & Agriculture 30, no 1-2, 38-46.
(Note that the references cited section will be single-spaced with a space between the “References Cited” and first reference as well as a space between each of the references.) -
