Develop two (2) separate social programs to reduce
recidivism and help individuals who were incarcerated reintegrate into society. Program
one is to reach out to these individuals so they know about this program and to
reach out to communities so they also know about this program and to give them
awareness and bridge them together where they can meet to be there for each
other, helping them to integrate into society.  Program two is a
counseling program for these individuals to help them with the problems that
caused them to be incarcerated in the first place and to help them integrate
into society by overcoming barriers. Counseling includes individuals, groups,
and families. Both programs need to be based on social work values. 
Each program needs to be two pages and should include
1)  a description of
the program design,
2) the need it addresses (include community data),
3) target population,
4) description of a similar program (preferably an
evidence-based or best practice) operating
locally, nationally, or internationally used as a guide in
design work,
5) goals/objectives, and
6) basic logic model with short, intermediate, and
long-term outcomes (not a chart).
