II. The Miracle on the Hudson – The Full Story/Simple Flying. about US Airways Flight 1549 that crash-landed on the Hudson River in 2009.
Include the following in your paper:
1- Describe how Flight Operations Quality Assurance (FOQA), FAA Aviation Safety Action Program (ASAP)/Pilot Safety Action Program (PSAP), and Line Operation Safety Audit (LOSA) could be used in the form of SMS assurance by the Chief Pilot of an airline. Use either a real example that has possibly caused an incident/accident or be creative and make one up.
2- Explain how Safety Risk Management (SRM) and Safety Assurance (SA) were or should have been applied.
