“Revamping a Resume for Transitioning from Corrections Nursing to Acute Care Nursing”

I have a resume that needs revised for a new position. I am a corrections nurse and I want to go into acute care nursing. 
I need to add my skills and qualifications to this resume as a corrections nurse, but I don’t know how to set the resume up so that it looks clean with all the information needed.
These are my skills and qualifications
